If you student is interested in participating in a sport this school year, here is some information you will need to know:

1. PIAA requires all students to receive a Sports Physical dated after May 1st. 
2. A completed PIAA Packet (pages 1-6) is required to be handed into the Athletic Trainer
3. Insurance is required to participate in sports. You can buy the school insurance to have your student-athlete covered.
4. If your student-athlete would like to play more than 1 sport, a Re-Certification form is required (page 7) for each additional sport.
5. If your student-athlete was injured during their first sport of the year, they are required to have a PIAA CIPPE Form or Athletic Trainer's form completed by a physician clearing them before participating in another sport.
